Service Description
###**EXAM(S) DETAILS:** *Written Exam:* - Written Exam Required. - Open Book Written Exam. - Allowed up to 2.5 Hours to Complete the Written Exam. *Proficiency Exam:* - Proficiency Exam Required. ###**COURSE OBJECTIVE:** To prepare the inspector to monitor and document drilled shaft operations resulting in safe foundation installation per the Contract Documents. ###**COURSE SUBJECT MATTER:** - Will familiarize the inspector with the terminology, equipment and process details pertaining to drilled shaft installation - Will provide the inspector with the knowledge and understanding of Foundation Plans, Standard Specifications (Section 455), related details of the Construction Project Administration Manual - The course describes the inspector's role from step one of the Drilled Shaft Installation Plan to the final step of shaft installation and acceptance - The course includes a review of the equipment used for all drilled shaft installation including miscellaneous shafts, methods of installation (dry, wet, temporary casing, and permanent casing); pilot holes and test holes; slurry and slurry testing; shaft bottom inspection; reinforcing steel and it's placement; concrete placement (tremies and pumps); load tests; tolerance; QC guidelines; acceptance; and pay quantities (homework and problems are included). ###**EXAM REQUIREMENTS:** *Initial Qualification Requirements:* - Pass Drilled Shaft Written Exam - Pass Drilled Shaft Proficiency Exam - Obtain on-site experience in which at least he/she has directly observed, inspected, and recorded the full installation of at least ten (10) shafts under the supervision of a CTQP qualified Drilled Shaft Inspector (DSI). - At least five (5) of the drilled shafts inspected shall be on shafts constructed using the wet method. The CTQP qualified DSI shall include a note in the first page of the Drilled Shaft Excavation log as evidence for the required experience, indicating the name of the trainee and stating the trainee has inspected the full installation of the shaft. - Trainees must confirm work experience when taking the CTQP Drilled Shaft Inspector Exam or report work experience through the CTQP website (IA Re-qualifica *Re-Qualification Requirement:* - Pass Drilled Shaft Written Exam only, prior to expiration date. CANCELLATION OR RESCHEDULE BY STUDENT FOR ANY CLASS: If a student cancels or reschedules prior to seven (7) calendar days before their scheduled class, a refund in the form of a credit will be issued for the purpose of rescheduling at a later date. The credit will expire one year after the date of the original class date. The credit expiration date is a onetime extension. If a student cancels or reschedules seven (7) calendar days or less before any scheduled class or does not show up to a scheduled class, a fee of 50% of the full price of the class will be deducted from the total paid and the remaining balance will be available in the form of a credit that will expire one year from the date of the original class date. Valid reasons for cancellations of any class or exam/proficiency would include, but not limited to, weather conditions or lack of student registration. If the decision to cancel any class/exam/proficiency is made, A&SW will notify the contact person responsible for registering the student via email or phone. A&SW will not be responsible for any costs incurred for lodging or travel expenses.
