###**EXAM(S) DETAILS:** *Written Exam:* - Written Exam Required. - Closed Book Written Exam. - Allowed up to 2 Hours to Complete the Written Exam. *Proficiency Exam:* - Proficiency Exam Required. ###**COURSE OBJECTIVE:** To safely and effectively sample soil materials from the roadway, take earthwork density readings and record those findings in an FDOT Density Log Book. ###**COURSE SUBJECT MATTER:** - Moisture Density Relationship (FM 5-521 and 5-525) - Density of Solid and Bituminous Concrete Mixtures in Place by the Nuclear Method (FM 1-T238) - Determination of Moisture Content by Means of a Calcium Carbide Gas Pressure Moisture Tester (FM 5-507) - Reporting Test Results in the FDOT Density Log Book, Sampling Soils, Limerock, Cemented Coquina, Bank Run Shell, and Stabilizing Materials from the Roadway (FM 5-504) - Nuclear Gauge Maintenance - Sampling Soils and Limerock ###**EXAM REQUIREMENTS:** *Initial Qualification Requirements:* - Pass Earthwork Level 1 Written Exam - Pass Earthwork Level 1 Proficiency Exam - Valid Nuclear Radiation Safety Certificate (Recommended: www.APNGA.com) ​ *Re-Qualification Requirement:* - Prior to expiration date, pass Earthwork Level 1 Written exam - Valid Nuclear Radiation Safety Certificate Allowing your Certification to expire, will require you to follow the Initial Qualification Requirements. CANCELLATION OR RESCHEDULE BY STUDENT FOR ANY CLASS: If a student cancels or reschedules prior to seven (7) calendar days before their scheduled class, a refund in the form of a credit will be issued for the purpose of rescheduling at a later date. The credit will expire one year after the date of the original class date. The credit expiration date is a onetime extension. If a student cancels or reschedules seven (7) calendar days or less before any scheduled class or does not show up to a scheduled class, a fee of 50% of the full price of the class will be deducted from the total paid and the remaining balance will be available in the form of a credit that will expire one year from the date of the original class date. CANCELLATION BY A&SW CONSULTANTS, INC.: If A&SW Consultants, Inc. makes a decision to cancel any class/exam/proficiency, a full refund of fees paid will be given in the form of a credit or refund check if desired. Valid reasons for cancellations of any class or exam/proficiency would include, but not limited to, weather conditions or lack of student registration. If the decision to cancel any class/exam/proficiency is made, A&SW will notify the contact person responsible for registering the student via email or phone. A&SW will not be responsible for any costs incurred for lodging or travel expenses.
