Service Description

###**EXAM(S) DETAILS:** *Written Exam:* - Written Exam Required. - Open Book Written Exam. - Allowed up to 2.5 Hours to Complete the Written Exam. *Proficiency Exam:* - Proficiency Exam not Required. ###**COURSE OBJECTIVE:** To assure job site concrete tests including quality control, quality assurance, verification, and dispute resolution tests for concrete construction, used as part of the acceptance program are performed in accordance with the Standard Test Methods, project specifications and other contract documents. ###**COURSE SUBJECT MATTER:** - FDOT Specifications on the Field Placement Sampling and Testing of Portland Cement Concrete ###**EXAM REQUIREMENTS:** *Initial Qualification Requirements:* - ACI Concrete Field Testing Tech Grade 1 Technician - Pass FDOT Concrete Field Specification Inspection Written Exam ​ *Re-Qualification Requirement:* - Same as Initial Qualification Requirements ###Concrete Field Qualification has two Levels, Level 1 & Level 2 *Concrete Field Tech Level 1 requires the following certifications:* FDOT Concrete Field Inspector Specification (class we offer, CTQP class) ACI Concrete Field Tech Recommended Training Course for ACI Certificate CMEC, http://www.cmec.org The Level 1 qualification is good for 5 years. Since both certifications above are required, the overall qualification expiration date is based on whichever above certification expires first. For example: FDOT CF class expires 1/1/15 and ACI CF expires 5/1/15. The expiration date will be 1/1/20. The student then needs to re-certify their FDOT CF class before 1/1/20, then the qualification expiration date will become 5/1/20, until the student re-certifies the ACI CF, then the overall expiration will become 5 years from the latest FDOT CF qualification. Whichever one expires first, that’s what the qualification overall expiration date is based on. *Concrete Field Tech Level 2 requires the following certifications:* FDOT Concrete Field Inspector Specification (class we offer, CTQP class)(same as Level 1) ACI Concrete Field Tech (Recommended Training Course through CMEC, http://www.cmec.org) (same as Level 1) ACI Construction Transportation Concrete Inspection (CTCI) (Recommended Training Course through CMEC, http://www.cmec.org)​ The Level 2 qualification works just like the Level 1, with regards to the expiration dates, but now there are three certifications to wat Class Material - Release 1 - January 2015

Cancellation Policy

CANCELLATION OR RESCHEDULE BY STUDENT FOR ANY CLASS: If a student cancels or reschedules prior to seven (7) calendar days before their scheduled class, a refund in the form of a credit will be issued for the purpose of rescheduling at a later date. The credit will expire one year after the date of the original class date. The credit expiration date is a onetime extension. If a student cancels or reschedules seven (7) calendar days or less before any scheduled class or does not show up to a scheduled class, a fee of 50% of the full price of the class will be deducted from the total paid and the remaining balance will be available in the form of a credit that will expire one year from the date of the original class date. CANCELLATION BY A&SW CONSULTANTS, INC.: If A&SW Consultants, Inc. makes a decision to cancel any class/exam/proficiency, a full refund of fees paid will be given in the form of a credit or refund check if desired. Valid reasons for cancellations of any class or exam/proficiency would include, but not limited to, weather conditions or lack of student registration. If the decision to cancel any class/exam/proficiency is made, A&SW will notify the contact person responsible for registering the student via email or phone. A&SW will not be responsible for any costs incurred for lodging or travel expenses.
